Individual Membership Pricing
No enrollment fees. No contracts.

Ages 4–19: $49/month
Requires an enrolled adult. Child-only membership: $69/month -
Ages 20–49: $79/month

Ages 50–64: $99/month
Patients age 65 and older who are enrolled in Medicare or Medicaid are not eligible for membership.
Family Membership Pricing
No enrollment fees. No contracts.

Family of 4 – $240/month
Includes 2 adults and 2 children under age 20. Additional children may be added for $35/month each

Single-Parent Family – $165/month
Includes 1 adult and 2 children under age 20. Additional children may be added for $35/month each
Billing, Cancellation & Re-Enrollment Policy
Members will be billed for the first and last month of membership at the time of enrollment.
A minimum of thirty (30) days’ written notice is required to cancel membership. Upon receipt of proper notice, the member’s services will continue through the final thirty (30) days, during which no additional charges will be incurred beyond amounts already collected.
To promote continuity of care and prevent misuse of the membership model for short-term or episodic services, patients who voluntarily terminate their membership and later request reinstatement shall be subject to a non-refundable re-enrollment fee of $150, payable prior to reactivation of services.
The practice reserves the right to modify membership policies with advance notice as permitted by law.
Non-Member Visits - $ 175
For patients who are not yet members, we offer same-day, 30-minute visits (such as sick visits or sports physicals) when our schedule allows.
